How to send your monthly report
log80 makes a monthly report you can print or save as a PDF. It lists each activity, the date, and the hours, next to the 80-hour goal. It has a line for you to sign and date. Your state Medicaid agency tells you where to send it and what else it wants.
This page is not legal advice. Check with your state Medicaid agency. Your state decides who needs to meet the 80-hour rule and what proof it takes.
What the report is
The report is your record, not a government form. Some states have their own form. Check your state page. You can also print a blank hours log.
Where to send it
There is no one national mailbox. Your state Medicaid agency says whether to upload it, mail it, or bring it to an office. Ask them. log80 does not send your hours to Medicaid for you.
Steps to send your report
- 1
Finish the month
Log every day in the 80 hours tracker. Add proof photos if you have them.
- 2
Download the report
Open your monthly report. Download the PDF or print it. Sign and date the last page if you use a printed copy.
- 3
Ask your state
Ask your state Medicaid agency where to send proof and whether it has its own form.
- 4
Send what they asked for
Send the report the way they asked. Keep a copy for yourself.
