Terms of Service

Log80 is a personal record-keeping utility. It helps you record work and other qualifying activities and prepare reports from information you provide.

Log80 is not a healthcare provider, health plan, Medicaid agency, or business associate of Medicaid. Log80 does not act for Medicaid or make decisions for any Medicaid program.

Log80 does not provide medical, legal, tax, benefits, or eligibility advice. The app cannot determine whether an activity qualifies, whether an exemption applies, or whether a report will be accepted. Rules differ by state and may change. Confirm your requirements with your state Medicaid agency or a qualified professional.

You must be at least 18 years old and able to form a binding contract to use Log80. You agree to provide accurate registration information, keep it up to date, and keep your sign-in details confidential. You are responsible for all activity that occurs under your account. Notify us promptly if you believe your account has been compromised.

Information you choose to enter may include personal details and health-related exemption information. You are responsible for the accuracy of your entries and for deciding what to store or share. Do not use Log80 to communicate with a doctor, seek emergency care, or store medical records that are not needed for your personal activity log.

You retain ownership of the information you enter. You grant us a limited license to store, process, and display that information solely to operate and provide the Service to you.

Log80 reports organize the information you enter. A timestamp, seal, or audit trail can help show when a record was created or changed, but it does not make the record true, guarantee legal compliance, or require an agency to accept it.

You agree not to:

• Enter false, misleading, or fraudulent information into the Service.
• Use the Service to create records intended to deceive any person or agency.
• Access another person's account or data without permission.
• Probe, scan, or test the vulnerability of the Service, or attempt to bypass security or access controls.
• Interfere with or disrupt the Service, its servers, or the networks connected to it.
• Copy, scrape, or harvest data from the Service by automated means without our written permission.
• Use the Service in violation of any applicable law or regulation.

We may suspend or terminate accounts that violate these rules, and we may report suspected fraud to the appropriate authorities.

The Service is provided "as is" and "as available." We may update, modify, suspend, or discontinue any part of the Service at any time, with or without notice. We do not guarantee uninterrupted, timely, or error-free access, and we do not guarantee acceptance of any report by any government agency.

Log80 is not liable for any disruption, outage, delay, data unavailability, or loss of access to the Service, including during maintenance, upgrades, or events outside our reasonable control. You are responsible for keeping your own copies of any records you may need for a deadline.

To the fullest extent permitted by law, Log80 disclaims all warranties, express or implied, including warranties of merchantability, fitness for a particular purpose, accuracy, and non-infringement. We do not warrant that the Service will meet your requirements, that reports will be accurate or accepted, or that the Service will be secure or free of errors.

To the fullest extent permitted by law, Log80 and its owners, officers, employees, and affiliates will not be liable for any indirect, incidental, special, consequential, or punitive damages, or for any loss of profits, data, goodwill, benefits, or eligibility, arising out of or related to your use of (or inability to use) the Service — including any decision by a government agency regarding your benefits.

Our total liability for any claim arising out of or relating to the Service will not exceed the amount you paid us, if any, in the twelve months before the claim, or one hundred U.S. dollars (US $100), whichever is greater. Some jurisdictions do not allow certain limitations, so parts of this section may not apply to you.

You agree to indemnify and hold harmless Log80 and its owners, officers, employees, and affiliates from any claims, damages, losses, and expenses (including reasonable attorneys' fees) arising out of your use of the Service, your violation of these Terms, your violation of any law or third-party right, or any information you submit through the Service.

You may stop using the Service and delete your account at any time from your account settings. We may suspend or terminate your access at any time for violation of these Terms or to protect the Service or other users. Upon deletion, your data is handled as described in our Privacy Policy. Sections that by their nature should survive termination (including disclaimers, limitations of liability, and indemnification) survive.

We may update these Terms from time to time. If we make material changes, we will post the updated Terms on this page and update the effective date. Your continued use of the Service after changes take effect means you accept the updated Terms.

These Terms are governed by the laws of the Commonwealth of Massachusetts, without regard to conflict-of-law rules. Any dispute arising out of or relating to these Terms or the Service will be brought in the state or federal courts located in Massachusetts, and you consent to their jurisdiction.

This End User License Agreement ("License") governs your use of the Log80 application and related services (the "App"). By downloading, installing, accessing, or using the App, you agree to this License. If you do not agree, do not use the App.

14.1 License grant

We grant you a limited, non-exclusive, non-transferable, revocable license to use the App for your personal, non-commercial record-keeping purposes, subject to these Terms. This is a license to use the App, not a sale. We and our licensors retain all rights, title, and interest in the App, including all software, design, text, and trademarks.

14.2 Restrictions

You may not:

• Copy, modify, distribute, sell, rent, lease, or sublicense the App.
• Reverse engineer or attempt to extract the source code of the App, except where such restriction is prohibited by law.
• Remove or alter any copyright, trademark, or proprietary notices.
• Use the App to build a competing product or service, or for any purpose other than your own personal record keeping.
• Use the App in any way that is unlawful, fraudulent, or harmful, or that could damage, disable, or impair the App.

14.3 Your content

You own the records and information you enter into the App. You are solely responsible for that content and for ensuring it is accurate and lawful. We claim no ownership over your content.

14.4 Updates and changes

The App may automatically download and install updates intended to improve or extend the Service. We may change or discontinue features at any time. This License applies to all updates unless an update is accompanied by a separate license.

14.5 No guarantee of outcomes

The App is a record-keeping tool. It does not provide medical, legal, benefits, or eligibility advice, and it does not guarantee that any report will be accepted by any agency or that you will receive or keep any benefit. Decisions about your benefits are made solely by your state Medicaid agency.

14.6 Termination of license

This License is effective until terminated. It ends automatically if you breach these Terms, and you may end it at any time by deleting your account and stopping all use of the App. Upon termination, you must stop all use of the App.

14.7 Disclaimer and liability

The disclaimers of warranties and limitations of liability in Sections 7 through 10 of these Terms apply fully to this License and to your use of the App.
